EVENT AGREEMENT UPLOAD

Due on or Before October 14, 2016

The Indy Crossroads Championships Event Agreement stipulates the expectations of each participating team. The agreement is to be read and signed by the head coach of the team.

  1. Download the official Indy Crossroads event agreement form.
  2. Read and sign the event agreement form.
  3. Save the completed event agreement to your computer.
  4. Fill in your team name and grade division below and upload your completed event agreement.
